The London Pathway (UK, 1138741) Browse events

YTD Raised: £0.00

Pathway supports homeless people with serious illnesses Homelessness in England is rising, but we're rising to the challenge.  Every year we support over 3000 patients through our 11 NHS teams, helping them get back on their feet, find accommodation, and deal with the problems that led to their homelessness. Taking care of your health is hard when you don't know where you'll sleep that night, or the next time you'll eat, which means homeless people end up in A&E six times more than people with a home. It's difficult for doctors too - a person who needs bed rest, nutritious food, regular medicine and someone to take care of them can't get that on the street. That's where Pathway comes in. Our teams of doctors, nurses, housing officers, legal advisors and social workers work together to help homeless people find accommodation. In some areas we have created specialist ‘respite’ service, where patients can go for a bit longer to recover and recuperate.Supporting doctorsWe train professionals to spot homeless patients and help them. We also carry out research into all aspects of health, homelessness and exclusion, to help improve medical practice and make services better for patients. We host The Faculty for Homeless and Inclusion Health, a network of over 750 people who are passionate about taking care of homeless and vulnerable patients.Creating a Life After HomelessnessOnly people who have been homeless can tell us how it feels. That's why Pathway includes formerly homeless people - Experts by Experience - in every aspect of our work. We also run the UK's only homeless Care Navigator Programme, training people who were once homeless to work in hospitals supporting others. It's a life changing project, but one that can only continue with support from kind donors. Please support us, give a little to change a lot.
