Hospital At Home is a home nursing service delivering up to 24 hour care to patients in their own homes. The service supports terminally ill patients who wish to die at home instead of in a hospital or hospice. The criteria for admissions is that the patient would otherwise be in hospital. The patient's own GP has to give support to home nursing. Full domiciliary care is offered by SRN's and trained staff. The service also offers care for post operative patients and rehabilitation. The service greatly assists families who wish to be on hand with the nursing of their loved ones in their own familiar surroundings. All necessary equipment is taken into the home where required. There is nothing for the patient/family to pay.

All our services are provided free of charge and we rely heavily on voluntary donations to continue providing our special care. Southern Area Hospice Services was founded in 1989 as a 6-bedded unit and has since expanded to become a 12-bed in-patient unit, which serves the entire Southern Health Board Region. The unit operates 24 hours a day throughout the year and is the heart of the hospice. Southern Area Hospice provides the service across the Southern Health Board, including Newry & Mourne, Craigavon, Banbridge, Portadown, Armagh and Dungannon. With a fundraising target of £2.38 million for 2015 all services are provided free of charge to patients and their families and include 12-bedded in-patient unit Out-patients clinics at Craigavon, South Tyrone and Daisy Hill Hospitals as well as St. Johns House, Newry. Age Concern Twyford and District is a small local, inderpendent charity which provides daycare and associated services to the elderly of Twyford, Berkshire and surrounding area. Our small, friendly day centre serves the local community in a warm and happy atmosphere. We provide a stimulating, caring and active environment, offering companionship as well as respite care. We aim to meet all needs and are committed to treat our members with the respect they deserve. For many members the Centre plays a vital role in improving their quality of life and it is almost like having an extended family. We can take up to 25 people a day; some members come just once a week and others attend nearly every day. Our members are transported in by volunteer drivers and the Readibus. Referrals come from the Health Centre, Social Services, Home Care, hospitals, friends and families. Some members are self referrals. The Centre is open five days a week. On three of the days we offer full day care facilities with a home-cooked midday meal and members can join in a wealth of activities such as quizzes, games and sing-songs. On the other two days we have lunch clubs for the more able bodied members.
